PAR-Q+ screening
Clients may be directed to the official PAR-Q+ screening process before beginning a program. FORME does not reproduce or administer the official PAR-Q+ questionnaire on this website. We may ask you to confirm that you completed the screening and whether it indicated that additional professional guidance may be appropriate.
Payment information
Payments are processed by Stripe. FORME does not directly collect or store your complete payment card information. Stripe processes payment information according to its own privacy and security practices.

We use information you provide to process your order, personalize your fitness program and applicable general nutrition guidance, communicate with you about your purchase, deliver your program, provide purchased support or check-ins, respond to questions, and operate and improve FORME.
