About FORME
FORME provides personalized digital fitness programming and, where included in the purchased package, general nutrition and wellness guidance. Services are delivered digitally and are designed using information provided through the FORME client intake.
Educational services only
FORME services are for educational and general wellness purposes only. They are not medical care, physical therapy, diagnosis, treatment, rehabilitation, or medical nutrition therapy.
You are responsible for determining whether exercise is appropriate for you and for following any recommendations or restrictions provided by qualified healthcare professionals.
PAR-Q+ and exercise readiness
Clients may be asked to complete the official PAR-Q+ screening before beginning a program. If the screening or a healthcare professional recommends additional evaluation, clearance, restrictions, or follow-up, you agree to follow that guidance before beginning activity beyond the level advised.
Your responsibility
You agree to provide accurate and complete information during the intake process and to inform FORME of relevant limitations or professional exercise restrictions that may affect programming.
You understand that participation in physical activity involves inherent risks. You are responsible for exercising within your abilities, using appropriate equipment and technique, and stopping activity if you experience concerning pain, dizziness, unusual shortness of breath, or other symptoms that warrant professional attention.
Personalized programs
Programs are created based on the information you provide at the time of purchase and intake. They are intended for the individual purchaser and may not be copied, resold, redistributed, or shared commercially without permission from FORME.
Delivery
Completed personalized plans are generally delivered by email within 2–3 business days after FORME receives a completed client intake containing the information needed to create the program. If you purchased 24-hour expedited delivery, your order will be prioritized for delivery within 24 hours after your completed intake is received.
Delivery may be delayed if additional information or clarification is required from the client.
Results
FORME does not guarantee specific fitness, body composition, strength, health, or nutrition outcomes. Results vary based on many factors, including consistency, effort, individual response, lifestyle, nutrition, recovery, and circumstances outside FORME's control.
Refund policy
Because FORME provides customized digital services that require individualized review and program creation, purchases are generally non-refundable once work on your personalized program has begun.
If you request cancellation before FORME has begun creating your personalized program, you may contact FORME to request a refund. Refund eligibility will be reviewed based on the status of your order and any applicable consumer rights.
If you receive the wrong product, experience a duplicate charge, or have a problem with delivery, please contact FORME so the issue can be reviewed and corrected.
Program changes and support
Revisions, check-ins, program adjustments, or ongoing support are included only when specifically listed in the package purchased. Additional changes or services may require a separate purchase.
Digital products and intellectual property
FORME programs, written materials, branding, graphics, and original content are owned by FORME or used with permission. Purchasing a service gives you a personal, non-transferable right to use the materials for your own fitness purposes.
Payments
Payments are processed through third-party payment providers. By making a purchase, you agree to provide accurate billing information and authorize the applicable charge for the service selected.
Limitation of liability
To the fullest extent permitted by applicable law, FORME is not responsible for indirect, incidental, special, or consequential losses arising from use of the website, services, or personalized programming.
Nothing in these Terms is intended to exclude rights or protections that cannot legally be waived.
Changes to these Terms
FORME may update these Terms as services or business practices change. The effective date above will be updated when material changes are made.
